Effects of Preoperative Instruction on the Reduction of Postoperative Anxiety and Recovery in Hysterectomy Patients


Abstract
The purpose of this study was to investigate the cause of preoperative anxiety and the preopera-ive state-anxiety, and to observe the effects of preoperative instruction on the reduction of postoperative stateanxiety, the incidence of postoperative complication in hysterectomy patients. .
From July 14 to Oct. 21, 1986, 43 patients were accepted according to the research criteria at 1 general hospital in Seoul; 21 patients were selected for the experimental group were instructed before operation; while 22 other patients in the control group were not instructed for the purpose of measuring and comparing the level of anxiety and the effect of preoperative instruction.
The results were as follows:
1. There was shown state-anxiety to be alleviated in experimental group than control group (p =.0)1). 2. The three main cause of preoperative anxiety about hysterectomy patients were pain and discomfort, recovery, destruction of body image.
3. Among the cause of anxiety, R2 for state-anxiety were as follows: loss of feminity was 33.92%, destruction of body image was 14.22%, pain and discomfort was 7.36%.
Above 3 main causes had 55.5% of R2 to the state-anxiety score.
4. Preoperative instruction did reduce the frequency of analyseics uses (p=.002) and resumption to me to natural bladder elimination (p =.004), and did reduce the frequency of postoperative complication (n=.047). but did not reduce the time of expulsion of flatus after oneration (n=.068).
